The ACC Adult Congenital Heart Disease Passport was developed by the Congenital Heart Disease and Pediatric Cardiology Committee to assist patients with a successful transition from pediatric to adult health care.
It is important for older children and teens to develop competancy in understanding the nature of their heart problem and the plan of managment.
As a first step toward developing this competancy, the parents are encouraged to keep a running history of important medical milestones and to gradually transfer this responsibility to the child in developmentally appropriate stages. This record or "Passport" can serve as a tool for facilitating this passage and a document as the emerging adult transfers care to an adult Cardiolgist. It is hoped that the Passport will form the basis for a life-long habit of playing a central, knowlegable role in the patients own healthcare.
