When applying for any membership category in the ACC or advancement to a higher category, please follow the procedures below.

Apply online or download a copy of the application at any time of year.
- Complete all sections of the application in order to avoid delay. Attach documentation as required.
- Applicants are required to obtain two letters of sponsorship. The letters must be on the sponsors’ business letterhead, signed by the sponsor and included with the application, otherwise, the application will not be processed or reviewed. Request letters of sponsorship from Fellows of the College who have reviewed your application. Sponsoring Fellows must be well acquainted with your current professional activities. At least one of the sponsors must be from your geographic area of professional activity; however, only one of the sponsors can be from a partner or colleague who works in your office. You are also limited to one sponsor from your training program. Relatives may not sponsor.
- Submit the completed application packet consisting of:
- Signed application form and attachments.
- Letters of sponsorship.
- Payment of annual dues (see under Annual Dues Schedule) and nonrefundable processing fee.
- Lacking board certification or applying through the academic/science track, or through the option for other Cardiovascular related subspecialties, your bibliography must be included with the application.
A completed application may be submitted online or a hard copy downloaded and sent to the Member Services Department at the American College of Cardiology, 2400 N Street NW, Washington, DC 20037 USA.
The Election Process
- Individuals currently in training are not eligible to apply for F.A.C.C., Associate Fellow or Affiliate membership.
- Elections occur regularly through the year. Some applications require review by the Credentialing & Membership Committee (CMC). The CMC meets to consider certain applications two times during the year.
- Candidates elected to Fellowship are invited to participate in the Annual Convocation of the College, which is held in conjunction with the Annual Scientific Session, usually in March. Certificates of Fellowship are distributed immediately following the Convocation.

New applicants must include payment for annual dues plus a nonrefundable application fee of $150. If advancing members have already paid dues, they need only include the application fee.
