COURSE OVERVIEW/STATEMENT OF NEED
The American College of Cardiology Foundation (ACCF) recognizes that cardiovascular medicine, especially the provision of optimal care for atrial fibrillation (AF), is increasingly managed across a diverse range of providers. Thus, ACCF is launching the Mind the Gap Forums which will allow cardiologists and primary care physicians an opportunity to develop next-generation collaborative strategies regarding AF and optimal anticoagulation. While information and knowledge is the first step in addressing this important issue, meaningful dialogue between referring primary care physicians and cardiologists is essential for practically managing anticoagulation in local communities. The Mind the Gap Forum will allow that dialogue to occur.
Learner Objectives Upon the completion of this session, attendees should be able to —
- Implement evidence-based anticoagulation regimens for AF patients based on individual risks and patients' preferences
- Recognize common barriers associated with managing chronic anticoagulation in AF patients

All relevant potential conflicts of interest that are identified are thoroughly vetted through a process that includes course directors and appropriate peer review by education committee chairs/members and/or peer review of content by independent, non-ACC-affiliated clinicians, for fair balance, scientific objectivity and validity, and patient care and safety recommendations.
